• Send a copy of the completed and saved form with a scanned sales receipt or warranty card via email to extended.warranty@leica-camera.com or
• Send a print out of the order form and a copy of the sales receipt or warranty card via regular mail to Leica Camera AG, Customer Care, Solmser Gewerbepark 8, D-35606 Solms
